Requirements

  • Must possess and maintain a current Registered Nurse license AND certification as a Nurse Practitioner with the state of California.
  • Must possess and maintain a current American Heart Association Basic Life Support (BLS) certificate.
  • Current/unrestricted DEA certification.
  • 2+ years of NP experience working in a medical office or hospital setting.
  • NP experience in assessing/managing those with substance use disorders, mental illness, and trauma care.
  • Experience with EMRs.
  • Ability to work independently with little supervision.
  • Cultural sensitivity and ability to work with a diverse patient population.

Responsibilities

  • Provide clinical support to the Street Medicine team.
  • Deliver direct medical care to unsheltered, homeless individuals with comorbid physical, mental health, and substance use disorders.
  • Work as part of a multidisciplinary team to develop treatment plans and provide ongoing care to high-risk homeless patients.
  • Participate in strategic planning for the growth and clinical enterprise of Street Medicine.
  • Facilitate relationships with other practitioners and learners.
  • Provide ongoing education to staff, learners, and the community on health and homelessness.
  • Develop new protocols for street medicine in collaboration with the director of clinic operations and medical director.
  • Establish standards for clinical application of new or established guidelines for patients.
  • Participate in local and state street medicine organizations.
  • Develop quality standards for clinical operation in inpatient and outpatient settings.
  • Oversee quality of patient care with input from the clinical program leader and medical director.
  • Ensure accurate tracking and dispensing of pharmaceuticals.
  • Maintain timely and accurate documentation.
  • Assist in the development of Policies and Procedures with input from directors of clinic operations and medical director.
  • Perform primary medical, obstetrical, and pediatric care to various patient demographics.
  • Conduct routine medical examinations and diagnose common medical conditions.
