The Community Skills Trainer will serve as a role model, group leader, and motivator, facilitating and assisting consumers in achieving goals related to wellness and daily living skills such as grooming, hygiene, nutrition, cooking, cleaning, and medication management. The role involves documentation as required by Valley HealthCare System, Medicaid, OSHA, and other licensing agencies, ensuring that all documentation is completed accurately and timely. The trainer will perform predetermined services as per the Individual Treatment Plan and complete Electronic Medical Record (EMR) documentation during each daily episode of care, noting the client's progression or regression in their individualized treatment goals. The position may require attendance at client Interdisciplinary Team meetings and participation in Treatment Plan development, while ensuring the safety, health, and well-being of consumers at all times. The trainer will implement treatment and behavior plans in compliance with Valley HealthCare System's and State guidelines, monitor and maintain equipment and supplies, and transport consumers to appointments and activities as needed. Additionally, the trainer will support the training of new employees and engage consumers in achieving their daily goals through active treatment.
