As a role model, group leader, and motivator, Community Skills Trainers will facilitate and assist consumers in achieving goals of wellness and daily living skills (i.e. grooming, hygiene, nutrition, cooking, cleaning, taking medications appropriately, etc.). Documentation as required by Valley HealthCare System, Medicaid, OSHA, and other licensing agencies. Employee ensures that documentation (such as Direct Services Progress Notes, Client Transportation Progress Notes, etc.) are completed accurately and timely and will reflect services the client(s) has received services as appropriate. Perform all-predetermined services as per Individual Treatment Plan. Complete Electronic Medical Record (EMR) Documentation during each daily episode of care, notating, clinically, client's progression/regression in each of the clients' individualized treatment goals identified in Master Treatment Plan. May attend client Interdisciplinary Team meetings and participate in Treatment Plan development. Assure that the safety, health and well-being of consumers are always maintained on or off site. Implement treatment/behavior/ plans in compliance with Valley HealthCare System's and State guidelines. Completes and submits program specific documentation in a timely and accurate manner. Monitor, maintain and use appropriately, all equipment and supplies. Transportation of consumers to appointments, activities in the community, and to/from Program site, using a company vehicle or employee's personal vehicle, on rare occasions. Support and help in the training of new employees. Attends in-service trainings, mandated meetings and completes Essential Learning trainings as assigned. Employee will engage consumers in achieving their daily goals and objectives through active treatment.
