About The Position

The Community Service Manager is responsible for planning, implementing, and supervising daily operations of the Community Services program, including the daily operations of the transportation department, and providing social work services to the Town of Duxbury residents (age 60+). Responsibilities include assessing older adults’ needs and strengths, providing information and assistance accessing services, advocating for and supporting older adults and their families and overseeing the logistics of the Center’s transportation services and supervising the transportation services staff. Employee is required to perform all similar or related duties.

Requirements

  • Bachelor’s degree with one to three (3-5) years related work experience with older adults in a home or community setting, or any equivalent combination of education, training and experience which provides the required knowledge, skills and abilities to perform the essential functions of the job.
  • Knowledge of elder services and resources.
  • Ability to interact in a positive and effective manner with people particularly older adults ability to communicate orally in a clear, and concise manner; ability to manage multiple tasks; ability to receive, understand, and execute oral, and written instructions; ability to maintain detailed accurate records, and to prepare written and oral reports.
  • The ability to relate to older adults in a comfortable fashion with genuine interest, compassion and concern for their needs and feelings. The ability to assess the needs and strengths of older adults.
  • Knowledge and demonstrated usage of personal computers particularly word processing and spreadsheet applications; excellent public relations, telephone skills and sensitivity to individual client issues.
  • valid driver’s license and pass a CORI/SORI and background check.

Responsibilities

  • Plans, coordinates and evaluates the Community Services program in accordance with the Duxbury Senior Center five-year plan and under the supervision of the Senior Center Director.
  • Provides social work support to other departments related to older adults’ concerns and services. Particularly, the Social Day, Home Delivered Meals and Transportation programs.
  • Provides general social work services, including assessing older adults’ needs and strengths, care plan options, information and referrals, advocacy, and support to older adults and their family members through home and office visits as well as telephone contacts. Provide follow-up as needed.
  • Facilitates and coordinates support groups at the senior center for older adults and family caregivers.
  • Designs strategies to identify and build relationships with socially isolated and/or frail older adults to access services, attain safety and autonomy and improve quality of life.
  • Compiles monthly and annual statistical reports and other data, as needed.
  • Builds and maintains good working relationships with community groups, older adult service providers, older adult housing property managers, and faith communities.
  • Works collaboratively with all departments in coordinating special educational programs for older adults.
  • Compiles up-to-date information about older adult programs and services. Information includes transportation, food, fuel assistance, financial and legal issues, adult day care and social day programs, support groups, assisted living and nursing facilities.
  • Represents Duxbury Senior Center on Committees and Coalitions as needed.
  • Assists older adults with applications and paperwork for public assistance programs.
  • Supervises the transportation department, including the coordinator and bus drivers. Oversee bus driver training and keep updated records.
  • Completes Gatra annual report on a timely basis.
  • Oversees the onboarding of Social Work Interns
  • Completes Monthly Report for Social Work interns.
  • Documents all activities related to assisting older adults, family members, and the general public.
  • Mandated reporter of elder abuse and neglect.
