About The Position

The Community Service Coordinator at Clover Health plays a vital role in supporting the telephone care coordination team, which includes RN Care Managers. This position focuses on assisting members in accessing community resources and providing administrative support for various clinical initiatives aimed at improving healthcare outcomes for members, particularly those with complex medical needs.

Requirements

  • High School Diploma and/or GED.
  • Bilingual in English/Spanish (strongly preferred).
  • Excellent customer service and communication skills (verbal and written).
  • Strong administrative and computer skills, especially with Google Apps (Mail, Calendar, Sheets, etc.).
  • Experience with social service resources and applications.

Nice To Haves

  • Professional experience in Medicare, Medicaid, or healthcare settings is a plus.
  • Experience with medical claims or past roles in healthcare provider settings.

Responsibilities

  • Provide coordination assistance for ongoing and new clinical initiatives such as medication adherence and readmission prevention.
  • Assist RN Care Managers with non-clinical follow-up tasks or care coordination, including setting up transportation and confirming fax receipts with provider offices.
  • Provide administrative support to clinicians by faxing, scanning, and mailing information.
  • Assist members in filling out applications for benefits like assistance programs and Medicaid.
