About The Position
Join our team as a Community Resource Specialist at our PACE Center in Topeka and collaborate with PACE Social Workers to connect our PACE Participants with vital services and support in our local communities. By partnering with local agencies and organizations, you will play a key role in improving the well-being of individuals and families in our community. Your work will directly impact the well-being of our participants by ensuring they have access to the support they need to live their best lives. Midland Cares Program of All-inclusive Care for the Elderly (PACE) is a not-for-profit health care program that helps people at risk for needing nursing home care who meet a level of care requirement stay in their own homes and communities.
