The Southcentral Foundation (SCF) Community Resource Specialist is responsible for performing work with clinic staff, customer-owners, and social resource sources regarding the coordination of a broad range of Federal, State, and Local programs, services, and resources as well as time-intensive care coordination tasks. They are able to function as part of multiple Integrated Care Teams and work with provider teams, referral sources, and customer-owners to improve the customer experience and overall quality of care. This position has two (2) levels designed to provide progressively more responsible and independent work experiences. Progression between job levels is based on the demonstrated ability to successfully handle more progressively responsible assignments.
