Responsible for securing information and completing yearly applications for Prescription Assistance for uninsured or underinsured patients. Additionally, this position assists patients to schedule medical transportation to outpatient appointments and procedures reducing missed medical appointment and to complete KanCare Medicaid and Social Security Disability Applications for eligible patients in the clinic setting. The goal of the Community Resource Representative is to assist clinic patients with financial assistance by obtaining unaffordable medications, securing transportation through free to no costs resources, and apply for state or federal funded insurance options. This position must professionally interact with patients, caregivers, physicians, other medical staff, pharmaceutical companies, insurance companies, state agencies, and employers to research and identify potential resources.

  • Research eligibility and requirements assisting patients to complete Prescription Assistance Applications through pharmaceutical companies.
  • Manages approved prescription applications for patient monitoring prescription changes, refills, and yearly renewals.
  • Assists patients with the completion of applications to qualify for government health insurance coverage or social security disability benefits.
  • Schedules medical transportation for patients to appointments or procedures through insurance programs, reduced bus fares, community resources, or organization's transportation.
  • Connects patients with community resources by addressing social determinant health needs to continue life in the community.
  • In collaboration with multi-disciplinary treatment team develops appropriate and supportive plan of care to address identified needs.
  • Consistently uses diplomacy and respect both in person and when using the telephone, and performs effectively and professionally under stressful conditions.
  • Develops professional working relationships with community resources on behalf of patient/family/caregiver.
  • Ensures customer concerns are processed in compliance with organization's policies while maintaining the highest level of patient and employee rights, including confidentiality of patient information and personnel issues.
  • Determines priority of daily work and effectively completes work as assigned.
  • Accommodates and supports the changes required to meet departmental and organizational goals and customer needs.
  • High School Diploma / GED Required
  • 1 year Customer service experience. Required
  • 1 year Health Care or Community Resources experience. Preferred
  • Ability to be adaptable to a variety of situations focusing on patient satisfaction. (Required proficiency)
