About The Position

Join a leader in community mental health with a vision for the future that sets the pace for associate self-care! The Community Navigator serves as a field-based resource who meets individuals and families where they are in their neighborhoods, community spaces, and at outreach events. This role focuses on connecting people to behavioral health services, social supports, and community resources, while reducing barriers to care. The Community Navigator provides direct assistance with benefits and entitlements, social determinants of health (SDOH) resources, and mental health/substance use disorder (MH/SUD) needs. This position conducts targeted outreach to veterans, Miami Indians, residents of housing projects, and other community groups while also engaging in general outreach and education efforts across the service area.

Requirements

  • Bachelor’s degree in social services, psychology, or related mental health field preferred.
  • Associate’s degree with two years’ experience, or high school diploma with five years’ experience in mental health, social services, or related field accepted.
  • At least 2 years of experience in a case management or community-based role.
  • Willingness to submit to a pre-employment drug screen.
  • Willingness to submit to comprehensive background checks, including but not limited to criminal and child protective services checks.

Responsibilities

  • Conduct field-based outreach to connect with individuals in community settings, including housing projects, veteran groups, schools, and local events.
  • Conduct thorough assessments of individuals' needs and resources to determine the best course of action.
  • Act as a liaison between individuals and community resources, such as healthcare providers, government agencies, and non-profits.
  • Serve as a visible and approachable presence at community events, community meetings, and gatherings to share resources and strengthen partnerships.
  • Provide on-site navigation support, including screening for MH/SUD needs and linking individuals to appropriate services.
  • Assist individuals with enrollment in benefits and entitlements (e.g., Medicaid, SNAP, disability supports) and connect them to SDOH resources (housing, food, transportation, utilities, etc.).
  • Collaborate with local organizations, tribal representatives, and veteran groups to address unique needs and expand access to care.
  • Develop contact and resource lists for communities.
  • Follow up with clients in the field to ensure successful connection to services and address barriers that may arise.
  • Maintain strong, proactive communication with internal staff and community partners to coordinate care and referrals.
  • Document all outreach contacts, referrals, and follow-up outcomes in compliance with agency and regulatory standards.
  • Respond to stakeholder communications within 24 hours of receipt.
  • Meet assigned productivity and outreach goals.
