The Community Resource Navigator is a healthcare worker who is a trusted member of and/or has an unusually close understanding of the community served. This role supports patient care in accordance with established methods, techniques, and standards of The Corvallis Clinic. This position will work and bill under the supervision of a licensed mental health provider, and report to the Pediatric Practice Manager.

Requirements

  • High School Diploma/GED (or higher).
  • 2+ years of experience in a behavioral health setting or 1+ years of experience in a healthcare setting or Certificated Health Worker (CHW) with the Oregon Health Authority.
  • Intermediate level of computer proficiency (including Microsoft Outlook, Teams) and be able to use multiple web applications.

  • Knowledge of Outpatient, ambulatory care and/or community healthcare.
  • Knowledge of Care coordination.
  • Knowledge of a primary care related setting.
  • Knowledge of Chronic conditions.

Responsibilities

  • Participate and maintain a culture within The Corvallis Clinic consistent with the content outlined in the Service and Behavioral Standards Caregiver document.
  • Coordinate with involved systems of care.
  • Assist with referrals.
  • Contribute to team care plans and planning.
  • Assist with transitions between providers and phases of care.
  • Connect people to community and/or social service resources.
  • Provide follow-up with patients and their families, if appropriate.
  • Deliver screenings and health education related to prevention and healthy behaviors.
  • Assist with mental health media outreach.
  • Provide resources for social support and build social networks.
  • Motivate and encourage people to obtain care and services.
  • Advocate for the needs and perspectives of patients.
  • Conduct two-way education about community resources.
  • Work with patients and families to teach them to interact effectively with the healthcare system.
  • Support the assessment, design, development, implementation, and evaluation of the Care Coordination Navigator Program.
  • Assist providers in creating and implementing new mental health initiatives for patients and trauma-informed clinical practices.
  • Perform other duties as assigned.
