This role is remote/work-from-home and visits to members' homes are required. Candidates should expect to travel 75% of the time and reside in the South Miami, FL region. Field-based work experience is highly preferred. Community health worker experience or similar is desired. The position supports community connection activities, including connecting members to community resources to support their care management journey and provide necessary care resources in a cost-effective manner. You will provide members with known community resources and support the care team to identify member community support and provide health education as appropriate. Additionally, you will support the coordination of community outreach resources available to members and promote awareness of care/services. As a community health worker, you will serve as support for members on community and care resource inquiries and opportunities available to them. You will also support all member-related correspondence and educational materials to assist in the facilitation of a successful community connection. Documenting and maintaining all community resources to ensure standards of practice and policies are in accordance with health plan requirements is essential. You will assist the clinical team of nurses and social workers with activities that include outreach, community education, informal guidance, and member support. Conducting non-clinical general health assessments will be part of your responsibilities to refer members to appropriate care/services, resolve concerns on members' behalf, and gather information for medical providers and staff working within the organization. You may also conduct non-medical assessments such as home safety, assessment of community/environment resources, transportation, employment, and others to be able to refer to appropriate care/services. Your role will involve telephonic and/or in-person outreach to locate individuals and families in the community who are hard to reach, and you may make visits to individual homes and/or community organizations. A working knowledge of Social Determinants of Health (SDOH) barriers is important, and you will perform other duties as assigned while complying with all policies and standards.
