As a Community Resource Coordinator II at Centene Corporation, you will play a vital role in transforming the health of our communities by connecting members to essential community resources that support their care management journey. This position is primarily remote, with the requirement to visit members' homes, necessitating a travel commitment of approximately 75% of the time. Candidates should ideally reside in the Miami, FL region and possess field-based work experience, particularly in community health or similar roles. In this role, you will support community connection activities by providing members with access to known community resources and assisting the care team in identifying additional support for members. Your responsibilities will include coordinating community outreach resources, promoting awareness of available care and services, and serving as a point of contact for members regarding community and care resource inquiries. You will also be responsible for documenting and maintaining community resources to ensure compliance with health plan standards and policies. Additionally, you will assist the clinical team, which includes nurses and social workers, by conducting non-clinical general health assessments and non-medical assessments such as home safety evaluations and resource assessments. This will enable you to refer members to appropriate care and services, resolve concerns on their behalf, and gather necessary information for medical providers. Your outreach efforts may involve telephonic and in-person communication to locate individuals and families in the community who may be hard to reach. A working knowledge of Social Determinants of Health (SDOH) barriers will be beneficial in this role, as you will be tasked with addressing these challenges to improve member outcomes.
