Summary Under direction of the Nurse Manager, Clinical Coordinator & Charge Nurse, the Community Residence Counselor (CRC ) at Lincoln Residence provides direct patient care and participates in therapeutic milieu within the scope of defined practice. Requirements

High School Diploma or Equivalent required
Experience working in a human services, residential, behavioral health, or allied health setting 1-2 years required
Experience working with individuals with mental illness, substance use disorders, or developmental disabilities 1-2 years required
Basic knowledge of behavioral health conditions and support strategies.
Strong interpersonal and communication skills, with the ability to maintain professional boundaries.
Ability to manage time, multitask, and respond to urgent needs calmly and effectively.
Commitment to person-centered care, equity, and cultural competence.
Familiarity with safety protocols and documentation standards in healthcare or social service environments.

Nice To Haves

Bachelor's Degree Psychology preferred
Bachelor's Degree Social Work preferred
Bachelor's Degree Health & Human Services preferred
Associate's Degree Related Field of Study preferred
Medication Administration Program Certified [MAP - Massachusetts] - Massachusetts Department of Public Health preferred

Responsibilities

Provides direct patient care in accordance with current treatment plan, under the supervisor of the Nurse Manager, Clinical Coordinator and Charge Nurse.
Provides and supervises community activities according to needs.
This may include driving & transporting patients to and from activities in hospital vehicles.
Remains awake and alert, at all times, while on the job.
Assists patients in maintaining and keeping various appointments.
Clear, concise, and timely documentation and shift notes.
Establishes relationships with patients that are helpful and maintains professional boundaries.
Participates in patient rounds, status checks, or teams per program guidelines.
Completes assigned patient care tasks per program guidelines and/or daily assignment sheet.
Works effectively to manage the therapeutic milieu.
Provides patients with direction and supervision as needed.
Follows established hospital & program policies, procedures, and protocols.
Communicates information regarding patient milieu/therapeutic environment to appropriate staff in a timely manner.
Communicates observations related to medication side effects, physical and emotional changes to appropriate medical staff in a timely manner.
Encourage patients in their activities of daily living.
Assesses sleep patterns and assists patients to develop more appropriate sleep cycles notifying clinical team as necessary.
Completes mandatory annual in-service requirements, including but not limited to: CPR and de-escalation.
Attends and participates in rounds and case conferences when appropriate.
Physical ability for participation in patient activities.
Other duties as assigned.
