Summary Under direction of the Nurse Manager, Clinical Coordinator & Charge Nurse, the Community Residence Counselor (CRC ) at Lincoln Residence provides direct patient care and participates in therapeutic milieu within the scope of defined practice. Requirements

  • High School Diploma or Equivalent required
  • Experience working in a human services, residential, behavioral health, or allied health setting 1-2 years required
  • Experience working with individuals with mental illness, substance use disorders, or developmental disabilities 1-2 years required
  • Basic knowledge of behavioral health conditions and support strategies.
  • Strong interpersonal and communication skills, with the ability to maintain professional boundaries.
  • Ability to manage time, multitask, and respond to urgent needs calmly and effectively.
  • Commitment to person-centered care, equity, and cultural competence.
  • Familiarity with safety protocols and documentation standards in healthcare or social service environments.

Nice To Haves

  • Bachelor's Degree Psychology preferred
  • Bachelor's Degree Social Work preferred
  • Bachelor's Degree Health & Human Services preferred
  • Associate's Degree Related Field of Study preferred
  • Medication Administration Program Certified [MAP - Massachusetts] - Massachusetts Department of Public Health preferred

Responsibilities

  • Provides direct patient care in accordance with current treatment plan, under the supervisor of the Nurse Manager, Clinical Coordinator and Charge Nurse.
  • Provides and supervises community activities according to needs.
  • This may include driving & transporting patients to and from activities in hospital vehicles.
  • Remains awake and alert, at all times, while on the job.
  • Assists patients in maintaining and keeping various appointments.
  • Clear, concise, and timely documentation and shift notes.
  • Establishes relationships with patients that are helpful and maintains professional boundaries.
  • Participates in patient rounds, status checks, or teams per program guidelines.
  • Completes assigned patient care tasks per program guidelines and/or daily assignment sheet.
  • Works effectively to manage the therapeutic milieu.
  • Provides patients with direction and supervision as needed.
  • Follows established hospital & program policies, procedures, and protocols.
  • Communicates information regarding patient milieu/therapeutic environment to appropriate staff in a timely manner.
  • Communicates observations related to medication side effects, physical and emotional changes to appropriate medical staff in a timely manner.
  • Encourage patients in their activities of daily living.
  • Assesses sleep patterns and assists patients to develop more appropriate sleep cycles notifying clinical team as necessary.
  • Completes mandatory annual in-service requirements, including but not limited to: CPR and de-escalation.
  • Attends and participates in rounds and case conferences when appropriate.
  • Physical ability for participation in patient activities.
  • Other duties as assigned.
