Involves work in community settings, including clients’ homes , supportive housing, and/or residential programs as well as in local and state hospitals. Assesses client needs, plans and links appropriate services, advocates for and monitors client needs to ensure the provision of comprehensive services to meet identified client needs. Coordinates and provides services in an integrated Individualized Recovery Plan (IRP). Ongoing interaction with individuals who have been diagnosed with severe and chronic mental illness and/or substance abuse diagnoses. All employees in this position are considered “Essential Employees” and may be required to provide mandatory overtime for program coverage needs.
