They will foster the highest possible quality of life and personal achievement in accordance with the individual’s person-centered plan. Essential Job Duties:A Children’s Community Provider is responsible for providing support services to youth with a range of emotional, behavioral and social difficulties. They will encourage growth and recognize the autonomy of each individual. It is recognized that the individual being supported drives the work and focus of this position. It is the individual’s home and their life choices. Attend paid new worker training and ensure regulatory training standards are met Providing required face to face services in the community or home, requires use of personal vehicle. Attend team meetings, supervision sessions, staff meetings as scheduled Form and maintain relationships with youth and caregivers Work with youth to achieve goals consistent with their service plans Written communication skills for documentation, including progress notes, correspondence notes, service plans, and safety plans which show progress toward individual goals, observations and interactions. Demonstrate competency with navigating electronic health record (EHR) for documentation of services rendered. Review utilization and service provision with supervisor Coordinate and collaborate with Care Managers to identify service needs and develop service plans Ensure appropriate chain of command is utilized. Comply with all federal and state regulations and agency policies pertaining to HIPAA, false claims, whistleblower policies and all others. Any other duty as assigned by your supervisor
