The Community & Patient Health Liaison will provide education, health promotion, outreach, and access to resources for patients and non-patients to increase healthcare access. This role will inform the public on services offered at Neighborhood Healthcare (Neighborhood) to reduce healthcare gaps in North County (Escondido, Poway, San Marcos, Valley Parkway). Additionally, this role will work in health centers and in the community to help build trust and awareness for Neighborhood. This role will be the point of contact between various community stakeholders, community-based agencies, and Neighborhood to increase patient growth and make critical linkages to offered programs and services. Must be open to work evenings and weekends to cover community events/meetings.
