About The Position

The Community Paramedicine Specialist role at UVA Medical Center focuses on enhancing health outcomes for underserved populations by connecting them with underutilized resources outside the traditional healthcare system. This position involves assessing patients in the community, facilitating access to healthcare resources, and addressing barriers to care, with the ultimate goal of improving health outcomes and reducing health disparities.

Requirements

  • Bachelor's Degree preferred or extensive prehospital experience in lieu of degree.
  • 3 years of experience in a relevant clinical environment or as a released Attendant-in-Charge with a Fire or EMS transport agency preferred.
  • Virginia Department of Health Emergency Medical Technician Intermediate (EMT-I) or Emergency Medical Technician Paramedic (NRP/EMT-P/Paramedic) required.
  • Valid Virginia Driver's License required.
  • American Heart Association (AHA) Health Care Provider BLS certification required.
  • Advanced Cardiac Life Support (ACLS) certification required.
  • Community Paramedicine Certification (CP-C) required within 12 months of hire.
  • Emergency Vehicle Operator Course (EVOC) Level II or higher required to operate a vehicle within 90 days of hire.

Responsibilities

  • Conduct patient assessments including vital signs and physical assessments as required.
  • Administer vaccines and obtain specimens for laboratory testing.
  • Provide wound care and emergency care within the scope of practice as a certified EMS provider.
  • Review electronic medical records (EMR) and documentation from recent healthcare visits.
  • Ensure patients can fill discharge prescriptions and attend follow-up appointments.
  • Review and ensure functionality of required healthcare equipment for patients.
  • Facilitate telemedicine opportunities and provide patient education.
  • Serve as a navigator between patients and various healthcare providers and resources.
  • Manage enrollment and assessments for various health surveys and tools.
  • Communicate regularly with patients to follow up on their progress and address barriers to care.
