About The Position

Total Care Connect (TCC) is a mobile integrated health organization delivering in-home clinical and preventive care to members across Ohio and surrounding regions. TCC supports health plans, health systems, and value-based organizations by reaching members where they are — in their homes and communities — to improve access, close care gaps, and reduce avoidable utilization. As a tech-enabled, field-based care delivery organization, TCC's teams provide a range of services including preventive care, chronic condition support, transition-of-care visits, member engagement, and navigation, operating with a focus on high-quality member experience, operational excellence, and coordinated care across clinical, administrative, and remote teams. The Community Paramedic delivers high-quality in-home clinical care as part of Total Care Connect’s Mobile Integrated Healthcare programs. This role focuses on proactive patient care for high-risk populations, supporting hospital readmission reduction, emergency department avoidance, and improved chronic disease management. Community Paramedics conduct scheduled patient visits, perform clinical assessments, deliver care within established protocols, and collaborate with virtual providers and interdisciplinary care teams to provide coordinated, patient-centered care.

Requirements

  • Current Ohio Paramedic certification in good standing
  • Minimum 2–3 years of paramedic field experience
  • Valid driver’s license with acceptable driving record
  • Strong clinical assessment and patient education skills
  • Experience using electronic documentation systems

Responsibilities

  • Conduct scheduled patient visits within community-based care programs.
  • Perform comprehensive patient assessments including vital signs, symptom evaluation, medication reconciliation, and environmental safety review.
  • Deliver care within established clinical protocols and standing orders.
  • Provide treatments such as wound care, medication administration, and point-of-care testing when applicable.
  • Identify early signs of clinical deterioration and escalate concerns through clinical or operational pathways.
  • Support continuity of care following hospital discharge or acute medical events.
  • Assist patients in understanding care plans and follow-up appointments.
  • Identify barriers to recovery including environmental or social determinants of health.
  • Collaborate with care coordination teams to support patient stabilization and long-term health management.
  • Complete accurate and timely documentation within the electronic patient care record system.
  • Ensure documentation supports clinical accuracy, medical necessity, and payer requirements.
  • Maintain communication with operational teams regarding visit updates and patient needs.
  • Participate in documentation reviews and quality improvement initiatives.
  • Perform other related duties as assigned.
