Requirements

• Basic Cardiac Life Support (BLS) certification.
• Certified Health Education Specialist (CHES) or Peer Support Specialist certification.
• Experience working with high-risk or uninsured populations.
• Spanish-speaking skills or proficiency in another language such as Russian.
• Computer proficiency, particularly with electronic health records.

Responsibilities

• Coordinate care for patients with the MHP Care Coordination team.
• Assist patients in accessing support services to achieve personal health goals.
• Act as a liaison between MHP patients, healthcare providers, and community services.
• Work with practices to meet network performance goals.
