The Community Outreach Program Manager (COPM) for Broome County plays a vital role in supporting REACH's mission to provide low-threshold, harm reduction-oriented primary care within a multidisciplinary, integrated practice. This position is responsible for ensuring health equity for all individuals who experience stigma within the current healthcare system. The COPM leads outreach for REACH in Broome County, overseeing mobile syringe exchange, harm reduction supply distribution, proper disposal, and compliance with data reporting. The COPM will supervise Community Health Workers (CHWs) who engage clients and deliver services such as connecting individuals to health and social supports, providing culturally appropriate education, assessment, case management, and ensuring timely assistance. The COPM may also directly provide these services when needed.
