Broome County Community Outreach Program Manager
About The Position
The Community Outreach Program Manager (COPM) for Broome County plays a vital role in supporting REACH's mission to provide low-threshold, harm reduction-oriented primary care within a multidisciplinary, integrated practice. This position is responsible for ensuring health equity for all individuals who experience stigma within the current healthcare system. The COPM leads outreach for REACH in Broome County, overseeing mobile syringe exchange, harm reduction supply distribution, proper disposal, and compliance with data reporting. The COPM will supervise Community Health Workers (CHWs) who engage clients and deliver services such as connecting individuals to health and social supports, providing culturally appropriate education, assessment, case management, and ensuring timely assistance. The COPM may also directly provide these services when needed.
Requirements
- Bachelor's degree or a minimum of five years’ experience.
- At least two years’ experience working with clients in health or human services.
- Experience navigating local medical and social support systems.
- Exceptional communication skills

Responsibilities
- Promote the mission, vision, and values of REACH.
- Deliver culturally appropriate support services to individuals in the community.
- Supervise, train, monitor, and remain available for regular and as-needed support to all REACH CHWs.
- Collaborate with other community-based organizations and CHWs to assess and address individual needs, especially concerning social determinants of health.
- Facilitate access to information, including support systems and educational materials in both web and print formats.
- Ensure that CHW referrals to external agencies are both appropriate and timely, with proper documentation supporting those referrals.
- Provide mental health screenings when needed, under the supervision of licensed behavioral health providers (with training provided as necessary).
- Participate in multidisciplinary team meetings, such as daily patient care pre-visit, huddles and regular collaborative meetings with community service providers.
- Provide ongoing supervision to CHWs, including conducting annual performance evaluations, setting goals, reviewing notes, and monitoring progress.
- Oversee operational aspects of grant deliverables.
- Learn and navigate the local housing and shelter systems within Broome County.
- Assist with building and maintaining progress reports for all community outreach programs, both grant-funded and non-grant-funded.
- Maintain focused and timely progress notes.
- Assists with the development and implementation of the organizations Syringe Service Program (SSP), including; documentation and reporting methods, promotional materials, ordering harm reduction supplies, inventory management and distribution.
- Train CHWs and oversee mobile syringe exchange services in surrounding counties that may include Broome, Cortland and Tompkins County.
- Train and supervise CHWs on Narcan distribution and overdose reversal protocol.
- Oversee harm reduction supplies and safe injection equipment distribution.
- Instruct CHWs to provide information on safer drug use, HIV/HCV prevention, overdose prevention, and referral to care.
- Provide care in the community with Broome County CHW’s on an as needed basis as part of training, supervision, and coverage of the area.
- Perform additional duties as assigned by the supervisor.
