About The Position

The Parker Jewish Institute for Healthcare and Rehabilitation, located in New Hyde Park, NY, is seeking a Community Outreach/Marketing Representative with a demonstrated healthcare marketing record, knowledge and understanding of Medicaid/Medicare guidelines and outstanding customer service and outreach skills, to join our team and play a key role in the successful growth and development Parker's Community Health Services Program-encompassing Parker's Certified Home Health Agency and Comprehensive Community Hospice. Parker Home Care is a Certified Home Health Care and Comprehensive Community Hospice, grounded on peerless experience and backed by respected leaders in the long-term care industry. Requirements

• Prior experience working in a community health setting.
• Knowledge of Medicare, Medicaid and MLTC guidelines/regulations.
• Two or more years of demonstrated experience in a Marketing, Outreach or Business Development role.
• College Degree.

Responsibilities

• Develop new referral sources in the community.
• Strengthen relationships with new and existing referral sources including daily visits to organizations, physician offices, hospitals, and assisted living facilities.
• Educate and inform referral sources and patients of home health options.
• Promote professional services within the community.
• Track and document all sales activity to ensure appropriate lead tracking and follow up.
• Bring workable patient referrals to the agency.
