As the Outreach Leader, you will be the face of Hopscotch Primary care in your assigned territory – ensuring that new patient growth calls for supported clinics are met on a monthly/quarterly basis. You will utilize your experience in territory sales management to effectively create demand for Hopscotch and create strong referral sources that lead to continued new patient growth success. In addition, you will partner closely with clinical team members to effectively strategize on new patient acquisition channels to ensure close alignment and visibility into growth performance.

  • Establish and maintain relationships with community partners in multiple counties to introduce, educate, and build demand for Hopscotch
  • Develop referral relationships within territory amongst external healthcare providers, insurance agents, and community organizations serving seniors
  • Plan, manage, and execute HPC events in the community (and in-clinic as needed) to drive new prospective patient awareness and demand
  • Implement daily, weekly, and monthly routing strategy based on business priorities and growth needs of each supported clinic
  • Partner closely with Marketing to ensure proper awareness of growth-specific events to aid attendance and new patient conversions
  • Effectively reach the Dual-Eligible population in designated territory to drive new patient growth amongst this population
  • Utilize Salesforce daily to effectively manage community partnerships and relationships with community stakeholders
  • Engage with inbound prospective patient leads (referrals) as quickly as possible to ensure the best possible customer and referral partner experience
  • Utilize Salesforce to develop, implement, and execute an effective prospective patient sales strategy and follow-up cadence to achieve patient acquisition targets
  • Perform daily outreach via email, phone, text, etc., to foster prospective patients
  • Collaborate with marketing to maximize effectiveness of prospective patient outreach and methods
  • Collaborate with Patient Engagement Representative team when necessary to ensure proper lead management and conversion success
  • Developing sales plans and forecasts over various time horizons (e.g., 3-month, 6-month) and measuring and reporting out progress to those plans
  • Following up with new patients to collect feedback and ensure their needs are being met
  • Tracking full-cycle outreach activities into Customer Relationship Management (CRM) system and other relevant business applications, from inputting new leads to successfully onboarding and retaining patients
  • Collect and document feedback from prospects and patients and adapt outreach to address challenges
  • Measure and report out on key sales and account metrics and performance to plan
  • Proactively communicate and strategize with clinical teams to ensure alignment and visibility into growth performance and priorities
  • Demonstrated success in managing a sales territory in previous roles
  • Possesses two to three years of healthcare sales experience
  • Has established relationships amongst the healthcare ecosystem and senior communities in designated territory
  • Is ambitious, organized, and energetic
  • Is empathetic and an active listener
  • Is a self-starter who is resourceful and doesn’t need a lot of direction to succeed
  • Has a drive to succeed and want to control their own income
  • Demonstrated ability to manage a high volume of daily outreach efforts (phone calls, emails, etc.)
  • Builds trust quickly and can handle tough conversations
  • Is collaborative and contributes to the effectiveness of the overall team
  • Demonstrates good judgement professionally and ethically at all times
  • Enjoys talking with seniors
  • Is comfortable using email, Microsoft Office applications, and computer reporting tools; CRM experience preferred
  • Has a valid driver’s license (required)
  • Has U.S. work authorization (required)
  • Degree from a 2- or 4- year institution
  • Two to three years of territory sales management experience
  • Two to three years of healthcare sales experience
