JOB SUMMARY This position will report to the Program Director and is responsible for creating partnerships and linkages, as well as identifying individuals in need of services for themselves or loved ones. The Community Outreach Case Manager will work a conventional work week that includes evening hours and may require weekend work. In identifying individuals in need of community-based suicide prevention efforts through. The Community Outreach Case Manager will work collaboratively with other organization to improve referral pipelines for referrals and care within VOACC's Suicide Prevention services.
