Northpointe is a Community Mental Health (CMH) organization providing services to individuals with serious mental illness, emotional disturbance, substance abuse disorders, and intellectual/developmental disabilities across Iron, Dickinson, and Menominee counties in the UP of Michigan. Northpointe itself was established in 1995 but has roots as a CMH dating back to 1964. Our presence can be found through remote emergency services, in school and home-based programs, and in clinical and residential home settings. Job Purpose We are looking for a passionate, driven individual to join our team as a Community Nurse serving Iron, Menominee, and Dickinson counties. This role is offered on a full-time hourly basis and does require on-the-job travel. The Community Nurse's primary function is to provide seamless, patient centered care for physical, behavioral, and social needs to vulnerable individuals with intellectual/developmental disabilities and/or serious mental illness who reside in Northpointe owned and operated Adult Foster Care (AFC) group homes and those enrolled in the Behavioral Health Home program. The Community Nurse works as a part of a multi-disciplinary integrated care team within our community to improve outcomes, access, and efficiency.
