About The Position

Provides hands-on nursing care to older adults in their homes and community settings, helping them remain safe, healthy, and independent. Working under the direction of an RN and as part of an interdisciplinary care team, the Community LPN monitors participants’ health, reports changes in condition, assists with implementing care plans, supports care coordination, and builds strong relationships with participants and families. This role combines clinical skills, independence, and teamwork to deliver participant-centered care outside of a traditional facility setting.

Requirements

• Current Licensed Practical Nurse (LPN) license in the state of Kansas
• Graduate of an accredited practical nursing program
• Ability to provide nursing care under the direction of an RN and within scope of practice
• Experience or comfort working independently in home and community settings
• Strong observation, documentation, and communication skills
• Ability to collaborate effectively with an interdisciplinary care team
• Valid driver’s license and reliable transportation for community visits
• Basic computer skills for electronic health record documentation

Nice To Haves

• Experience working with older adults, chronic conditions, or geriatric populations
• Previous experience in home health, community health, PACE, or long-term care
• Familiarity with care coordination or interdisciplinary care models
• Knowledge of PACE philosophy or person-centered care approaches
• Strong relationship-building skills with participants and families
