About the Role Ounce Community Navigators in Tennessee provide hands-on, resident-centered support within affordable housing communities. You will serve as a consistent, trusted presence onsite, helping residents access services, navigate benefits, improve health and wellness, and stay connected to the community. This role involves high-touch resident engagement, structured assessments, benefits navigation, crisis intervention, and strong coordination with property management and local partners. In this role, you will be a: Resident Support & Services Coordinator Conduct intake assessments, ongoing resident needs assessments, and ADL/IADL updates. Assist residents with health-care navigation, including scheduling appointments, coordinating equipment, and supporting follow-up after hospital or rehab stays. Provide benefits and entitlement support, including SNAP, SSI/SSDI, Medicare/Medicaid, and other local assistance. Deliver daily living support such as help with bills, phone/Internet setup, donations, paperwork, and lease education. Health, Wellness & Social Connection Facilitator Coordinate and host wellness events, health screenings, and provider-led workshops. Organize socialization activities such as crafts, group meals, bingo, or community gatherings. Support residents experiencing isolation through consistent outreach and engagement. Monitoring & Follow-Up Specialist Maintain regular follow-ups with residents and service providers to ensure needs are met. Complete wellness check-ins and home visits as needed. Resident Advocate & Crisis-Response Partner Support residents through urgent or complex situations, including health crises, safety concerns, and behavioral-health needs. Advocate for residents with property management, health systems, and partner organizations. Community & Provider Relations Liaison Build relationships with local service providers, senior services, food resources, mental-health agencies, health clinics, churches, and nonprofits. Coordinate onsite events, resource fairs, and partnership programming. Documentation & Compliance Steward Maintain detailed service logs, assessments, follow-up documentation, and event records in Ounce’s technology platform. Participate in quality-assurance reviews and ongoing training.
