About The Position

The Community Living Specialist 3 - AFC Case Manager is responsible for providing direct outreach services to individuals with disabilities, helping them achieve greater independence and self-fulfillment. This role involves developing and maintaining positive relationships with consumers to support their life goals. The position is full-time, based in Worcester, MA, and requires a commitment to work Monday through Friday during standard business hours.

Requirements

• Bachelor's degree with at least 2 years of experience working with elders or individuals with disabilities.
• One year supervisory or human service experience preferred.
• Knowledge of and respect for diverse populations.
• Valid driver's license with reliable vehicle.

Nice To Haves

• Fluency in Spanish preferred but not required.
• Experience in human services, residential care, direct care, or day programs.

Responsibilities

• Provide direct outreach services to individuals with disabilities.
• Develop and maintain positive working relationships with consumers.
• Assist consumers in achieving self-fulfillment through their life goals and desires.
• Transport members in a personal vehicle as needed.
