About The Position

The Center for Health Equity & Community Wellness (CHECW) seeks to eliminate racial and other inequities resulting in premature mortality. With an unwavering grounding in history and structural analysis, CHECW works to increase visibility of the harm perpetuated by centuries of racist, socially unjust policy while pushing towards redress for the most impacted NYC communities. CHECW addresses inequity across community and healthcare systems in partnership with community, faith-based, and health care organizations. CHECW's work focuses on social determinants of health, including environmental and commercial determinants, and addresses both upstream and downstream factors to improve health and well-being of New Yorkers. CHECW is comprised of the Bureau of Bronx Neighborhood Health, the Bureau of Brooklyn Neighborhood Health, the Bureau of Harlem Neighborhood Health, the Bureau of Chronic Disease Prevention, the Bureau of Health Equity Capacity Building, the Bureau of Equitable Health Systems, and the Bureau of Finance, Administration and Services. The division's Deputy Commissioner also serves as the Agency's Chief Medical Officer. The Bureau of Brooklyn Neighborhood Health will continue to provide oversight and leadership in programming and planning for the Brownsville Neighborhood Health Action Center with satellite programming and planning also occurring at the Bedford and Bushwick District Health Offices. This bureau will support programming, planning and research opportunities to address racial inequities resulting in premature mortality in Central Brooklyn. Reporting to the Bronx Family Wellness Suite Manager and working in partnership with other prenatal health programs out of CHECW and other BNH programs that address the social determinants of perinatal health, the Bronx Community Linkage & Referral Liaison will be primarily responsible for managing and expanding the stakeholder engagement and linkage programming for the Bronx Family Wellness Suite (FWS).

Requirements

  • A baccalaureate degree from an accredited college and two years of experience in community work or community centered activities; or
  • High school graduation or equivalent and six years of experience in community work or community centered activities; or
  • Education and/or experience which is equivalent to the above.

  • Hold certification as a Certified Lactation Consultant or have interest in pursuing this certification.

Responsibilities

  • Develop comprehensive portfolio to support stakeholder engagement and linkage referrals.
  • Build and maintain relationships with Bronx hospital, clinical, mental health and social service providers.
  • Lead in the management and facilitation of the Birth Equity Advisory Board.
  • Oversee outreach activities that promote awareness of the Bronx Family Wellness Suite programming.
  • Facilitate referrals into the Citywide Doula Initiative and other perinatal health programs.
  • Manage all promotion and recruitment into the stakeholder engagement programming for the FWS.
  • Maintain records of the stakeholder engagement and linkage referral portfolio of programming.
  • Support the administrative needs of the stakeholder engagement and linkage referral programming portfolio.
  • Develop appropriate continuous quality improvement and/or evaluation tools for all stakeholder engagement and linkage referral programming.
  • Support the promotion of other suite programming portfolios.
  • Support successful client intake, referrals to services, and management of appropriate client management software.
  • Provide support in increasing utilization of the FWS lactation room and other lactation clinic programming.
  • Support other Bronx and expanded FWS programming as necessary.
  • Other duties as assigned by supervisor and FWS leadership.
