About The Position

The Community Life Coach is responsible for helping people with intellectual disabilities connect to their communities through such activities as community events / activities and volunteer opportunities. The CLC assesses skills and interests for participants, implements service plans, and facilitates the involvement within the community. The CLC assures safety in the community while securing activities that enrich the participant’s life.

Requirements

  • High school diploma or G.E.D. required. Must be 18 years of age, or older.
  • Must be knowledgeable in Microsoft office and have the ability to use electronic charting programs.
  • Must have a valid drivers’ license and a good driving record as defined by AbleLight policy.
  • Upon hire, the CLC must successfully complete the Community Life Coach training.
  • Clear background check as defined by regulations and policies.
  • Ability to lift at least 50 pounds. Ability to do daily standing, bending and lifting.
  • Must be able to stand for 8 hours at a time..
  • Ability to establish and maintain professional and cooperative relations with local businesses and agencies.

Responsibilities

  • Deliver person centered care and services to those we are blessed to serve. Follow established plans created in collaboration with the person we support, parents & guardians, external clinical or local/state representative & providers, internal partners in quality, and operational leadership.
  • Identify community resources and activities pertaining to the interest and abilities of those we serve. Facilitate the participation in typical and integrated activities, events, and organizations in the local community in ways like other people of the same age and interest. Accompany participants to activities, if needed, and facilitate involvement and maintain or enhance personal independence.
  • Communicate progress, successes, and challenges to guardians / care providers as needed.
  • Ensure personal safety of participants on caseload. Maintain electronic records including progress of goals identified on participant’s person-centered plan, incidents, challenges, etc.
  • Help with tours and orientations of new clients and their guardians to programs.
  • Provide personal care and/ or other support as necessary (i.e., toileting, medication assistance, etc.)
  • Tracks and documents all attendance and transportation, providing that data to the Manager for billing. On an as needed basis she revises all pickup/drop off routes. Daily, checks the phone for any messages/client call offs and communicates that to the team so that route adjustments are made.
