About The Position

The Community Liaison Worker Level II - Outreach Specialist at NYC Health + Hospitals is responsible for supporting the delivery of high-quality care coordination services. Requirements

• Minimum of one year experience in outreach, sales, or customer service; or in a healthcare or health plan organization.
• Two additional years of full-time experience in counseling, community work, or community health activities in a government agency or community organization.
• Proficiency in Microsoft Office Suite (Outlook, Word, Excel).
• Strong written and verbal communication skills.

Responsibilities

• Conduct MLTC/MAP assessments for home visits.
• Manage referral assignments and administrative functions for care coordination programs.
• Perform telephonic outreach to schedule intake appointments for care coordinators.
• Complete Health Home HML assessments and review patient notes for billing accuracy.
• Review discrepancies in HARP reports and make necessary corrections in the EMR system.
• Complete required trainings relevant to the responsibilities of the position.
• Perform other duties as assigned.
