About The Position

PeaceHealth is seeking a passionate Community Liaison to help expand access to passionate care across our Home Health, Hospice, and Palliative Care services. In this dynamic role, you'll build relationships with referral sources—including hospitals, skilled nursing facilities, and assisted living communities—while educating healthcare professionals and patients about our comprehensive care offerings.

Requirements

• Bachelor's degree or equivalent experience
• At least 3 years' experience building business relationships in home care, hospice, or healthcare outreach
• Strong communication and presentation skills
• Proven success in sales or marketing
• Proficiency with Word, PowerPoint, and CRM tools
• A collaborative, results-driven mindset with the ability to work independently

Responsibilities

• Identify and engage with potential referral partners within your assigned territory
• Serve as a key connector between providers and PeaceHealth's Home and Community Care teams
• Create and execute outreach programs to grow awareness and increase patient referrals
• Deliver education to patients, families, and providers on PeaceHealth services
• Collaborate with internal teams to identify growth opportunities and improve access
• Maintain accurate records of outreach activity and referral partner engagement
