Community Liaisons caregivers concentrate their efforts on being exceptional communicators, assisting patients with transitions of care. In this role, you will also manage communications with care managers and post-acute care service providers and recommend marketing strategies based on customer relationships and trend data. A caregiver in this position works from 9:00AM - 5:30PM.
