Please Read Entire Job Posting Before Applying This is an In Office On Site Position This role operates in a Day Habilitation (DAY HAB) Setting , defined as services “that provide opportunities and support for community inclusion and build interest in and develop skills for active participation in recreation, volunteerism and integrated community employment” (Iowa Health & Human Services). Day Habilitation provides assistance with acquisition, retention, or improvement of socialization, community participation, and daily living skills. Community Integration Programmers provide a fresh look to the services we provide. As a core team member, you’ll be responsible for monitoring services, creating new strategies to reach goals, and coordinate services in a way that empowers people to meet and exceed their goals. In this role, you’ll be responsible to:

Create and maintain the team culture, which may include: o Being actively present and involved with your team. o Conducting monthly team meetings focused on immediate needs and team culture.
Function as Imagine’s main point of contact with the individual-served Medicaid Community Based Case Manager.
Developing Individual Service Plans.
Conduct quarterly reviews of goals/process.
Communicate with individuals served, families, guardians, and case managers in a timely manner regarding changes in services.
Overseeing resources, which may include: o Coordinate transportation for individuals served. o Monitoring budgets for individuals served are being maintained and followed as outlined in their financial plan by the Representative Payee Coordinator. o Supporting individuals in communication with and fulfilling responsibilities associated with their Representative Payee Coordinator. o Overseeing balances, ledgers, receipts, and special requests forms in collaboration with the Representative Payee Team. o Monitoring medication check-ins. o Monitoring appointment scheduling and follow-up communication for individuals served.
Ensuring services are provided to individuals in a way that honors their needs, desires, and goals, which may include: o Serving as Imagines' primary point of contact with the Medicaid Community Based Care Manager (CBCM) for individuals served. o Inputting accurate information regarding the individual served into the approved Electronic Medical Record (EMR). o Developing individualized plans with the CBCM and Interdisciplinary Team (IDT) that prioritizes the individual’s needs and goals. o Updating essential documents and ensuring authorizations are accurate. In addition, this role may require assessments to be completed for specific services or individuals. o Conducting quarterly reviews of goal progress for each individual-served under your team’s care. o Communicating with individuals served, their families/guardians, and case managers in a timely manner regarding any changes in the services provided. This includes changes to the kinds of services provided, a limitation of rights, or other concerns regarding the individual served. In this role, you will also be responsible to document these interactions and reasons for changes. o Collaborating with the IDT to write support goals and a personal outcome plan within 21 calendar days of a new individual being entrusted to our care. o Ensuring resources, such as ongoing supports, transportation, or other vital components to services provided, are available. o Collaborate with the Strategic Advancement team to promote local community partnerships and promote services offered by Imagine.
Agreeing to be placed on-call as required and fill in for the team if there is need. In this role, you must be available on short notice to fill gaps in the schedule due to absences, personnel changes, or other unexpected reasons. Compensation for on-call duties will be paid in stipends based on rotation, per department guidelines. Mileage reimbursement is available when on-call duties require report to work outside of regularly scheduled hours.
Demonstrate knowledge, or be willing to learn, and comply with all policies, practices, laws, and rules provided by any governing bodies in the industry.
Provide complete, consistent, and accurate documentation of incident reports, investigations, and service documentation.
Be an advocate for individuals receiving services to have as much control over their own lives as possible. All individuals should be treated with respect and should feel empowered to live their life as independently as physically possible.

Age Requirement: This position requires you to be at least 18 years of age.
Driver’s License: You’ll be responsible for transportation at times, so we need to trust your ability to be safe behind the wheel.
Reliable Transportation: You’ll need reliable transportation. You will also need to provide proof that your automobile insurance covers $100,000/$300,000/$50,000 liability coverage – don’t worry, we can answer any questions you might have about this.
Educational Requirement: Minimum Education Requirement of High School Diploma/GED equivalent.
Work Authorization: Candidates must be able to work in the U.S. without sponsorship
May require lifting at times, up to 35 pounds. Lifting more than 35 pounds requires the use of a two-person lift, mechanical lift, or assistive equipment.
May require prolonged sitting and working at a computer for up to eight hours.
May require frequent bending, stooping, and stretching.
May require prolonged standing for up to eight hours.
Must wear hearing and eye protection in required areas.
Must have ability to see objects/persons at a distance.
Must have ability to see close work such as typed or handwritten material.
Must have ability to hear conversations in a quiet and a noisy environment.
Must have ability to determine where a sound is coming from.
Must have ability to hear differences among bells, buzzers, beeps, horns, etc.?
Must have ability to communicate through speech.
Must have ability to drive and transport for service provision if needed.
Must remain awake during working hours, if applicable.

Competitive Wages: The base pay is $20.43/hour. With education and experience, you could start out making more than that.
Scheduling: This full-time position operates Monday through Friday, 8:00 a.m. to 4:30 p.m. As part of a 24/7 service organization, the role includes participation in an on-call rotation and may require flexibility to provide additional coverage as needed in our site homes.
