About The Position

Community Options, Inc. is a national non-profit agency providing services to individuals with disabilities in 12 states. We are actively seeking Part-Time Community Integration Counselors in Greater Phoenix, AZ. This role provides individualized services designed to assist participants in managing and overcoming the difficulties confronted by an individual with a disability living in the community. The hourly rate is $15.50 per hour! Location: Mesa, AZ Part Time Friday/Saturday/Sunday 9:00AM-7:00PM Can be a combination of Fri/Sat or Sat/Sun

Requirements

High School Diploma or GED
Valid driver’s license with a satisfactory driving record
Minimum 90 days of relevant experience supporting individuals with intellectual and developmental disabilities
Ability to establish a comfortable and supportive relationship with individuals receiving supports
Ability to exercise good judgment and remain calm in crisis situations

Responsibilities

Support and encourage individuals to increase their ability to be as independent as possible
Meet with waiver participants on a regular basis either in the individual’s residence, the community, or an office setting
Collaborate with the Case Manager to develop and facilitate programs which meet the social, emotional, physical and personal needs of the individual
Provide ongoing support for community integration efforts
Document individual’s progress toward acquiring community integration
Assist in building a support network between the individual and the community
Ensure medical consultation forms are completed, by the physician, for each medical appointment attended
Administer medications and ensures proper completion of necessary documentation
Coordinates all leisure and volunteer opportunities
Monitor the health and medical needs of individuals and immediately report any concerns
Drive individuals to medical appointments and activities in the community
Ensure program documentation and billable records are completed accurately and timely
