Job Summary The Community Integrated Case Manager provides case management services in shelter or housing programs and serves as the liaison between the CBI Shelter and Housing Programs and the CBI PCMH clinic. The Community Integrated Case Manager assists the patient and treatment team in planning, connecting, outreaching, advocating, coordinating and monitoring the patient’s needs and progress while in the program. This position also assists patients in gaining access to health services, financial assistance, employment, education, social services and natural supports as well as developing independent living skills. The Community Integrated Case Manager assesses patient needs, develops and monitors individualized treatment plans and documents services provided in an accurate and timely manner.
