About The Position
The Community Health Worker (CHW) at Henrietta Johnson Medical Center plays a vital role in connecting vulnerable residents of Wilmington, Delaware, to essential community resources. Under the supervision of the Chief Operating Officer, the CHW will employ culturally sensitive strategies to engage community members, promote wellness, and address Social Determinants of Health. This position focuses on outreach, education, and interventions aimed at improving health outcomes for priority populations, particularly in areas with high Social Vulnerability Index scores.
