About The Position

The Community Health Worker serves as a vital liaison between patients, care teams, and the community, focusing on removing barriers to health and social services. This role involves conducting outreach, assessing patient needs, and facilitating access to resources, ultimately aiming to improve patient health outcomes and well-being.

Requirements

• High School diploma or equivalent.
• Experience utilizing electronic medical records.
• Minimum of 1 year experience in a healthcare, community-based, or social work environment.

Nice To Haves

• Bilingual preferred.
• Certified Community Health Worker preferred.

Responsibilities

• Meet patients in clinics, facilities, or at home to identify social determinants of health affecting their well-being.
• Collaborate with social workers and care managers to develop action plans for patients.
• Build trusting relationships with patients by providing support and advocacy to help them achieve their health goals.
• Assist patients in accessing health-related services and community resources, including accompanying them to specialist appointments.
• Facilitate communication between patients, families, colleagues, and community organizations as needed.
• Document interactions with patients in medical records and on their behalf.
• Attend meetings as required and travel to various work sites as necessary.
