About The Position

The Community Health Worker (CHW) position at IMPaCT Care is designed for individuals who are natural caregivers and are passionate about helping others. This full-time role involves providing social support, navigation, care coordination, and advocacy for older adults in the community. CHWs will work closely with clients to enhance their health and well-being by addressing barriers to good health and connecting them with necessary resources.

Requirements

  • High School Diploma, GED, or equivalent (preferred)
  • 1 - 2 years of experience in a customer-facing role
  • Knowledge of the communities in which you will be working
  • Successful completion of initial and ongoing training including compliance training and CHW certification
  • Ability to communicate effectively in written and verbal forms
  • Strong interpersonal, problem solving, advocacy, and evaluation skills
  • Ability to bend, climb, stoop, and stand for an average of 5 hours per day
  • Ability to lift 30 pounds
  • Possesses a valid driver's license and car insurance
  • Possesses a valid second form of ID, such as a social security card
  • Must have reliable transportation
  • Willing to maintain skills through continued education
  • Must be able to pass a background check and drug testing
  • Must have a smartphone

Responsibilities

  • Meet with assigned clients in clinic, hospital, or home
  • Build and maintain professional and supportive relationships with assigned clients
  • Get to know assigned clients and help them improve their health
  • Conduct in-person client work including required home visits and assessments
  • Attend meetings with CHW Supervisors and provide updates on client cases
  • Serve as a link between the community and health/social services
  • Facilitate access to social services and support
  • Improve quality and cultural competence of service delivery
  • Motivate and teach clients to build wellness skills
  • Adhere to company policies including dress code and time management
  • Clock in and out of every shift appropriately
  • Observe clients for changes in behavior or health and notify personnel
  • Assist with administrative responsibilities including admissions paperwork and training new caregivers
  • Perform other duties as assigned
