About The Position

The Community Health Worker role assists patients in accessing resources/services, supports disease self-management, and coordinates culturally-appropriate care and services.

Requirements

• High school diploma or GED preferred.
• Valid Driver's license, clean driving record, and personal vehicle for use required if driving is required for the role.
• Two years of experience working in a social service or health care setting or equivalent educational setting preferred.
• Must be a resident of the community being served with good knowledge of the resources of their community.
