About The Position

Under supervision, this position assists the Integrated Care Team (Primary Care Physician, Nurse/Medical Assistant, Behavioral Health provider, Care Coordinator I, Care Coordinator II, etc.) by proactively coordinating care for patients to facilitate the achievement of quality and cost-effective health outcomes.

Requirements

• Must be at least 18 years of age.
• High school diploma or GED and minimum one year experience in behavioral health, social services, health care and/or education.
• Self-starter with strong verbal and written communication skills.
• Comfort with change and quality improvement processes.
• Experience with Electronic Health Records strongly preferred.
• Experience with Motivational Interviewing strongly preferred.
• Knowledge of New Mexico and local community resources strongly preferred.
• Must obtain within 180 days of hire and maintain current throughout employment: Certification as a Community Health Worker in the State of New Mexico.
