About The Position

A Community Health Worker is essential in addressing health disparities and improving health equity by providing personalized care and support to the clients. They act as a bridge between healthcare providers and the community, helping to ensure everyone has access to the healthcare services they need. The position requires the employee to complete issued trainings and additional follow-up training to maintain compliance. Other duties include representation of San Antonio Birth Doulas and Catholic Charities in the community.

Requirements

  • Education ? High School Diploma or equivalent
  • Experience ? Community Health Worker certificate or at least two years of work with a health care provider or community based organization
  • Caseworker or medical assistant preferred
  • Familiarity with concepts and philosophies such as informed consent, evidence-based practice, trauma-informed practice.
  • License and Credentials ? Must be at least 21 years of age
  • Reliable transportation
  • Valid driver license and clean driving record
  • Valid registration
  • Valid vehicle insurance
  • Must pass annual background checks
  • Excellent interpersonal skills
  • Knowledge and ability to navigate Zoom, Microsoft Office 2010 or more recent productivity suite, including MS Word, MS Excel, and MS Outlook, MS Forms, TEAMS, SharePoint to include touch screen and “smart” devices
  • Demonstrate knowledge of, and sensitivity to, the needs and interests of clients and their families
  • Demonstrate organizational skills and ability to multitask
  • Ability to maintain appropriate ethical boundaries and professional relationships with clients, doulas, and staff
  • Understanding of the unique position and needs of a community-based doula program
  • Bilingual preferred (English/Spanish)
  • Extensive working knowledge of pregnancy and childbirth;
  • A solid grasp of evidence-based practices and informed consent;
  • Must be detail oriented, organized, self-motivated, work well independently and on a team;
  • Must have good written and verbal skills;
  • Must have good critical thinking and problem-solving skills.

Responsibilities

  • Provide information on health topics such as nutrition and chronic disease management.
  • Conduct workshops and group sessions to educate the community.
  • Offer one-on-one education and support to individuals and families.
  • Offer emotional and social support to clients, addressing barriers to care.
  • Conduct basic health screenings, such as blood pressure checks.
  • Assist individuals in accessing healthcare services and resources.
  • Help clients understand and follow healthcare providers' instructions.
  • Schedule appointments and provide reminders for follow-up visits.
  • Organize and participate in community events and health fairs.
  • Distribute educational materials and promote health awareness.
  • Prepare reports and document outcomes to valuate program effectiveness.
  • Must be sensitive to the service population’s cultural and socioeconomic characteristics.
  • Adhering to safety training and protocols on a daily basis, and taking precautionary measures to ensure the safety and well-being of self and others.
  • Responsible for protecting the confidentiality of any information or material obtained in the service with the organization to include but not limited to client names and information, services rendered to clients, donors names and gifts, internal and external investigations or results of any investigations, and financial information.
  • Adhere to the Code of Conduct of the agency and the code of ethics and standards of doulas.
  • Every employee is required to take a solution-oriented approach in their interactions and undertakings, and work as a team member that promotes collaboration and commitment to the Mission and Vision of the organization.
  • Other duties as assigned by Doula Coordinator and Program Director to fulfill the goals and mission of San Antonio Birth Doulas and the needs of Catholic Charities.
