About The Position

Family Health Services is a Federally Qualified Health Center (FQHC) that provides quality, affordable primary care, dental, and behavioral health services. Our "high touch, high tech" clinics offer unique patient focused care to anyone in the community, regardless of their ability to pay. We are rapidly growing in Sandusky, Ohio at our two locations as well as in Norwalk, Ohio and surrounding areas. We also offer behavioral health and primary care at several local schools in hopes of eliminating barriers to those needing services. We are currently seeking a passionate Community Health Worker for Erie County OH. This role works in collaboration and continuous partnership with chronically ill, "high-risk", and/or pregnant patients and their family/caregiver(s), clinic/hospital/specialty providers and staff, and community resources in a team approach to promote timely access to appropriate care, increase utilization of preventative care and reduce emergency room utilization and hospital readmissions. This includes symptom monitoring, coordination and assistance in mental health interventions, and working in conjunction with behavioral health team to aid in the delivery of services for patient success.

Requirements

  • Current and unrestricted Ohio driver's license is a must.
  • Knowledge about local connections to community health care and social welfare resources.
  • Applicant must be organized and able to prioritize assignments
  • Must be able to do multi-tasking, problem-solving, and have decision-making skills
  • Follow detailed instructions with attention to detail
  • Ability to work with a culturally diverse group of people
  • Must be proficient in computer knowledge - Microsoft Office, Excel
  • Excellent typing skills
  • Knowledge of medical terminology
  • Good customer relationship skills and a strong work ethic
  • Applicant must possess independence and initiative

Nice To Haves

  • Preferred associate degree in Social Work, Counseling, Nursing or similar program.
  • Preferred Licensure of Qualified Mental Health Specialist.
  • Preferred 2-3 years of experience in clinical or community resource setting or care coordination/case management.
  • EMR experience a plus

Responsibilities

  • General office skills.
  • Serve as the contact point, advocate, and informational resource for patients, care team, family/caregiver(s), payers, and community resources.
  • Work with patients to plan and monitor care.
  • Assess patient's unmet physical and emotional health and social needs.
  • Create and promote adherence to a care plan, developed in coordination with the patient, primary care provider, and family/caregiver(s).
  • Increase continuity of care by managing relationships with patient, health care providers, and referral sources.
  • Educate patient and family/caregiver(s) about relevant community resources and assist them in accessing resources as needed.
  • Facilitate and attend meetings between patients, family/caregiver(s), care team, payers, and community resources, as needed.
  • Promote awareness of our services and identify ways to overcome any access barriers
  • Plan and execute creative activities with our local seniors to engage their mind, body, and spirits.
  • Work with the Director of Public Relations and the Nutritionists to coordinate group activities for our seniors, utilizing the rooftop area of the new medical center.
  • Be a liaison with the high rises and the senior center to work in collaboration with them.
  • Attend the monthly activities clubs at Harborview and Viewpoint
  • Work with and participate in wellness fairs, particularly those pertaining to senior wellness.
  • Work with our Director of Public Relations and Digital Marketer to design any print work to be displayed, passed out, hung on doors, or posted online to promote our organization and any outreach that is planned.
  • Work with our Director of Public Relations and Digital Marketer to create social media posts depicting outreach events that showcase what we are doing in the community.
  • Performs other duties as assigned.
