The Community Health Worker (CHW) is responsible for acting as bridge between health care providers, community services providers, and individual in the community, to promote health, reduce disparities, and improve services delivery. The CHW is a trained and trusted public health worker who is respected by the people they serve, and applies his/her unique understanding of the experience, socio-economic needs, language, and/or culture of the communities served. Having a unique understanding of the populations and the community they serve, CHWs spend 80% of their time doing outreach, health education, and navigation in the community settings. CHWs identify barriers to health and advocate for changing service delivery and other strategies to improve community health. WHAT YOU CAN EXPECT Monday to Friday- 8am to 4:30pm Will be covering Marion, Johnson and Morgan Counties Performs SDoH Screenings within Epic Attends community events and assist with building relationships in identified counties Link individuals to community resources. Collaborates with care management teams on patient services and screenings Engage patients and families onsite and community setting
