About The Position

The Community Health Worker is a key member of the Healthy Living Team who supports individuals and families through health education, outreach, and connection to resources to improve overall health outcomes. The role focuses on delivering the Diabetes Prevention Program to priority populations and recruiting and enrolling participants in YMCA chronic disease prevention programs to support long-term physical health and well-being.

Requirements

  • A high school diploma or equivalent is required.
  • Candidates must have an existing Community Health Worker certification or agree to complete Community Health Worker training provided by the YMCA and obtain certification within the first six months of employment.
  • Completion of all position specific and mandatory YMCA training within specific training deadlines.

Nice To Haves

  • One year of experience leading or facilitating classes and working with special populations is preferred, along with knowledge and experience in basic health and wellness principles, healthy living behaviors, and health behavior change.
  • Experience supporting participants through lifestyle changes, providing clear verbal instruction, ensuring classroom safety, and maintaining confidentiality.
