This is a per diem, hybrid position offering flexibility to work anywhere from 0 to 40 hours per week, depending on departmental needs. The schedule-consisting of 2 remote days and 3 days either in the field or onsite-will be determined by the hiring manager based on operational priorities. Please note that per diem team members are not eligible for health benefits; however, this role provides an excellent opportunity to gain valuable experience, contribute to meaningful work, and enjoy a flexible, dynamic schedule. Please note: We can only consider applicants who are able to commute to our Los Angeles work location a few times per week. If hired, you must reside within a commutable distance. The Community Health Worker (CHW) is responsible for establishing rapport and meeting with melanoma cancer survivors and family/friend members through in-person and virtual meetings. The meetings will involve education about melanoma prevention and early detection, support for motivating healthy choices, and guidance about self-examinations of the skin. The Community Health Worker will serve as a health navigation/resource bridge between the health education participants and health services. The CHW will maintain relationships to promote prevention and healthy behaviors and provide valuable resources to improve melanoma education and health outcomes. The Community Health Worker supports patients and their social and health needs by helping them navigate and access community resources, social services, and medical systems. Serves as a liaison between Cedars-Sinai and the community to facilitate access to services through assessment, care coordination, and other case management activities. The Community Health Worker is community based but will often partner crossfunctionally with Cedars-Sinai team members, including but not limited to, medical providers, social workers, and case managers to provide intensive personal support to patients to ensure that all health needs are met.
