General Summary Under the direction of the Director of Care Management, the Community Health Worker (CHW) helps patients access and navigate community services. The CHW conducts outreach, provides information and support to community members, and assists in data collection to identify health needs and promote process improvement, with a particular focus on high-risk populations.

Requirements

  • Associate degree in any field.
  • Basic Life Support (BLS) must be obtained within first 90 days of employment (BLS training is provided).
  • Certification as Community Health Worker must be obtained within first 6 months of employment.
  • Considerable mental concentration for sustained periods of time with frequent interruptions.
  • Light lifting (15 lbs.) required.
  • Standing, sitting, walking, reading, and speaking required.

Nice To Haves

  • Experience with social services or public health systems preferred.

Responsibilities

  • Establishes and maintains relationships with community partners, patients and their families while providing general healthy behavior support.
  • Assists families, groups, and communities in accessing both social and health related services.
  • Assesses client’s needs and refers clients to community resources and other agencies to meet identified needs, developing processes for assessing Social Determinants of Health in all settings.
  • Attends community meetings and appropriate events (ex: health fairs, etc.) to understand community issues and provide information to community regarding possible resources.
  • Advocates for individual and community health needs.
  • Acts as a liaison between Phelps Health, other community-based organizations and the community they serve.
  • Develops referral process and follow-up for identified patient needs.
  • Distributes flyers, brochures, and other informational pieces appropriate for the target population in the community served by Phelps Health.
  • Performs other related duties as assigned to support goals.
