About The Position

Good Samaritan University Hospital is a 437- bed teaching hospital and a member of Catholic Health. Our reputation for excellence is built on great choices and empowered leadership, and fosters a team that’s focused on evidence based practice, continual learning and exceptional quality of care The Community Health Worker is responsible for identifying high risk prenatal and postpartum women and assist these individuals in gaining access to primary and prevent2ve health services including prenatal care, PCAP, WIC, and other essential and supportive services as needed.

Requirements

  • High School Diploma or equivalent.
  • Valid New York State Driver's license, personal car to carry out employment duties and an active automobile insurance policy.
  • Basic computer skills.
  • Good writing and verbal skills.

Nice To Haves

  • Spanish or Creole speaking a plus
