About The Position

A Community Health Worker contributes to AHF by delivering comprehensive prevention and health services that address both medical and educational needs, particularly for communities facing systemic barriers such as LGBTQ+ individuals, Latine communities, youth, and people who use substances. Through their direct work providing care, facilitating testing and treatment, offering education, and linking clients to essential services, they build trust and empower people to take charge of their health. Their compassionate and culturally responsive approach helps reduce disparities and advances AHF’s mission to promote health equity and dignity for all.. A Community Health Worker enhances the reputation of AHF by serving as a trusted and compassionate representative within the community. Through their culturally responsive approach and dedication to meeting people where they are, they build strong relationships and foster trust in AHF’s services. By empowering individuals with knowledge, connecting them to care, and providing respectful, high-quality support, they reinforce AHF’s reputation as an organization committed to health equity, dignity, and the well-being of those most impacted by HIV . The Community Health Worker will work as a part of a close knit team to ensure clients receive seamless, comprehensive care and support. By collaborating with prevention staff, clinicians, and outreach teams, they help coordinate services and share insights that strengthen program impact. Their teamwork fosters an environment of mutual respect, open communication, and shared purpose, allowing the department to effectively reach communities, reduce health disparities, and advance AHF’s mission together.
