Position Summary This will be a field-based position working with Aetna members and Community Based Organizations (CBO) in Detroit (West) and Western Wayne county (Canton, Dearborn, Garden City, Inkster, Livonia, Northville, Plymouth, Wayne, Westland). Community Health Workers (CHW) serve the community as a bridge between the member (community) and the health care system (providers) through face-to-face outreach and education. By providing these community and home-visiting services, CHW's help Aetna members attain and maintain better health outcomes, improve relationships with healthcare providers and help both parties become more acculturated with each other. This position will work under the direction of the CHW Supervisor.

Requirements

  • Must reside in the Western Wayne area (Canton, Dearborn, Garden City, Inkster, Livonia, Northville, Plymouth, Redford, Wayne, Westland).
  • Must be willing to travel within the 50-90 mile radius within the assigned areas at least 70% of the time.
  • 1-2 years relevant experience (such as community outreach or home visits with members/patients).
  • 1-2 years of experience in serving the Medicaid populations in urban or rural environments with familiarity of local, formal, and informal resource networks.
  • High School Diploma or G.E.D.

Nice To Haves

  • Experience working in community/outpatient setting.
  • Community Health Worker Certification or equivalent training and experience.
  • Preferred skills: Mental Health First Aid, Trauma Informed Care, Motivational Interviewing, and specialized training in medical terminology and practices for designated health conditions.
  • Experience using a computer including the use of Excel and Electronic Health Records.
