About The Position

The Community Health Worker (CHW) is a frontline public health worker who is a trusted member of and/or has strong ties and a close understanding of the community served. This trusting relationship enables the CHW to serve as a liaison/link/intermediary between health/social services and the community to facilitate access to services and improve the quality and cultural competence of service delivery. As a “Community Health Worker” you will: Delivers services to vulnerable and high-risk individuals, particularly those hard-to-reach and/or with social needs, in home and community-based settings. Build relationships within the community (i.e., faith organizations, community groups/centers, community leaders, Housing Opportunities Commission, community businesses, etc.) Provides basic education regarding chronic health conditions and health promotion in a way that promotes understanding and self-management by the individual. Reinforces health literacy and educational messages using appropriate tools and supplies within scope of practice.

Requirements

  • Comprehensive knowledge of the local community.
  • High school graduate or GED required.
  • Minimum of two years' community involvement experience (i.e., community health worker, volunteer for community-based organization, outreach worker, community navigator, etc.)
  • Ability to effectively present information in one-on-one and small group situations to clients, and other employees of the organization.
  • Ability to communicate verbally and in writing and to hear or communicate with target population, medical and social service professionals, colleagues, and peers.
  • Ability to inspire and motive others.
  • Proficient with Microsoft Office applications including Word, PowerPoint and Excel

Nice To Haves

  • Certified Community Health Worker, preferred.
  • Motivational interviewing skills, preferred.
  • Bi-lingual skills to work with patients in their native language are preferred.
